How do I refill my prescriptions?

  • Most prescriptions can be refilled directly by your pharmacy if you have been seen by one of our providers in the preceding 12 months. The easiest way is to call your pharmacy and the will fax us our send us a request electronically. Please allow 3 to 4 days for processing.

I have HMO insurance can you see me?

  • We do not accept HMO insurance for payment of services. Some HMO policies do have a benefit for seeing a non HMO doctor. We are happy to bill your insurance for you or give you a superbill that you can send in to your insurance, but payment would be due at time of service. Any reimbursement that would come to the office would be sent back to you.

How much does an office visit cost if I don't have insurance or I have a HMO?

  • Your initial office visit would cost $120 and any follow-up visits would cost $100. If you would require any procedure or pathology the cost of that would be additional and you will be told an amount prior to consenting to the procedure.

I don't have my insurance card can i still be seen by the doctor?

  • We need to be able to copy the front and back of the actual card. If you do not have the card you can get a copy faxed or emailed to the office or you can pay $120 for the office visit and then turn in a superbill to your insurance company for reimbursement.

I am under 18, can you treat me without a parent present?

  • It is always best to have your parent present at all examinations but we do realize that for some of our teenagers that drive the parent does not want to miss work or is just not available sometimes. We need a note signed by your parent giving us permission to examine and treat you without their presence.
